*There is a $100 deposit for new patients using insurance. This deposit will go toward copays or anything insurance does not cover. If insurance covers completely, the $100 will be refunded to the original method of payment.
*We do not accept Medicaid.
*All copays are due at time of service.
*A re-exam fee may be charged if it's been over a year since you've been treated in our office and a new patient visit may be needed if it's been three years or more since you've seen us.
